Completing the Microsoft Excel Form

  1. Browse the tabs of the Excel document for instructions and form content
  2. Enter requested information on the first tab, and on each tab that contains a form you intend to submit
  3. Print the form(s) needed
  4. Get required signatures
  5. Scan and/or save a copy of the signed form(s) for your records
  6. Email a scanned (or pdf) copy of the form(s) to the address at the bottom of each form.
  7. If you are submitting an RF SIRI Financial Data Access form, the system user will need to complete the electronic RF confidentiality agreement available at:
